The form examples on pages 171-174 seems to be incomplete.
* The suggested html code on page 171-172 contains different " signs, which generates question marks if you try to copy/paste it into a form from the pdf (for example, the select name="Gender" has wrong quotation marks)
* A </select> is missing from the html on 171-172
* On page 173 two fields are discussed for mailinglist subscription which aren't present in the suggested HTML-code on page 171-172.
